Eye Service survey – ends 9th September
Crawley CCG and Horsham and Mid Sussex CCG , as part of an Alliance Initiative, are currently undertaking a transformation of our eye services, in order to ensure that patients receive timely diagnosis, treatment and support at the right place and by the right person. It is really important to us to understand what you, as a service user, think about current services and how they might be improved. This survey is anonymised.
The results of this survey will help us understand your eye health needs so that we can make improvements to community and hospital eye services in your area. Link to survey https://www.surveymonkey.co.uk/r/EYENP
